Right to Choose Adult ADHD Assessment

Please complete the below form if you are interested in a Right to Choose ADHD Assessment on the NHS for yourself as an adult (or if you are 17 1/2 years old).
Please read our eligibility criteria to confirm you are eligible. Once this form is complete, please press SUBMIT and we will email you a copy.

Please make an appointment with your GP, and take your Registration Form with you to discuss. You will need a GP referral to activate .
