Right to Choose ADHD Assessment

If you have a GP based in Hereford and Worcestershire, you are welcome to request a Right to Choose referral on the NHS. 

STEP 1: Ask your GP to complete a referral for Midlands ADHD Clinic via the Electronic Referral System (ERS)

STEP 2: We will send you an email containing intake questionnaires. Please complete these as soon as possible. 

Once we have received your completed questionnaires, we will review and triage your referral and add you to our waiting list.

Please be aware that delays in completing your questionnaires may result in a much longer wait before we can offer you an assessment date.
